Athletic Registration Process
SPRING 2025 REGISTRATION IS OPEN
Begin the registration process by logging into Final Forms:

All students who would like to participate in a spring sport must sign up on Final Forms. Following are the sports we offer in the spring and their start dates:
BOYS BASEBALL (M, V)
BOYS LACROSSE (M, JV, V)
GIRLS LACROSSE (M, V)
GIRLS SOFTBALL (M, JV, V)
BOYS TENNIS (V)
BOYS TRACK & FIELD (M,V)
GIRLS TRACK & FIELD (M, V)
COED UNIFIED BASKETBALL (V)
BOYS AND GIRLS GOLF (V)
TRAP CLUB (V)
ESPORTS (V)
Any parent/guardian of a student who participated in a spring sport simply needs to do the following on FinalForms:
Select the spring sport your child would like to participate in
Simply re-sign any forms which are red (you do not need to re-fill information that you already completed in the spring)
Make sure your child logs in and signs any forms which are red
Any parent/guardian of a student who is new to athletics needs to do the following:
Locate the parent icon and click "New Account"
Type your name, date of birth, and email and click "Register"
Check your email and confirm your email account
Create your password and click "Next"
Click "Register Student" for your child
Fill out all forms that are red - your form set is not complete until all forms are green

How to register for a sport:
Signups for any Varsity, JV or Modified athletics program (grades 7-12) will now take place on FinalForms, an online forms and data management service that we utilize in our athletics department. Final Forms allows you to complete and sign athletic participation forms for your students. Annual forms and student data will be saved from season-to-season and year-to-year, reducing the amount of times you need to fill out the same information to sign up your student.
Once your student is in FinalForms, parents/guardians will be able to track practice/game attendance, and will receive automated email reminders when physicals and forms are due. In order to participate in a sport in our district, a student-athlete must have:
An updated physical no longer than one year before their current sports season
Updated interval health form during each season per NYS regulations
Signed Code of Conduct and Attendance Policy
Signed Concussion Acknowledgement and Dominic Murray SCA Prevention Act
Viewed and signed Parent Athlete Meeting (now virtual through FinalForms)
First day to sign up for each sports season:
Fall: August 26, 2024
Winter: November 13, 2024
Spring: March 17, 2025
Parents/guardians of all athletes must use FinalForms to register each child.
Ensure your child has a valid physical on file with the district. If a physical and/or update is needed, click on the sports physical icon below for further information.
Physicals are good for a period of one year from the date of the physical, to the end of the month (i.e. physical done 8/03/18 is valid until 8/31/19) with a completed update form.
If the physical is more than 30 days from the start of the season, a parent/guardian must complete the health update form (which must be dated within 30 days of the start of the season
